GMS Registration Update

GMS Attestation letters must be submitted to the DCJS GMS Help Desk if any company or individual is acting on behalf of an applicant/grantee. The attestation should be similar to the below language, on organization letterhead, and signed by the applicant/grantee GMS signatory.

I, [Insert Name], as the signatory for [Insert Organization Name], give full permission to [Add Company Name or Individual’s Name], to act on behalf of and make decisions for my organization during the application process and for any subsequent contract that may be awarded as a result of said application.

GMS Attestation letters must either contain a wet signature from the organization’s signatory, or the letter must be submitted to DCJS directly from the signatory’s email address. This request also applies to any/all current contract(s) with which any outside company is affiliated. This will ensure all parties are in agreement with the terms and conditions of the application process and/or contract. Please combine all GMS Registration forms and GMS Attestation letters into one email per organization.
